The goals listed in your charter fit my style perfectly. I am looking to get back into raiding but I want to play in a relaxed atmosphere. I would be more than willing to heal on a regular basis but I would need some practice.

To get comfortable with raiding again and to have lots of fun.

Minimal. Made Marshal back in Vanilla on my Rogue but now I just stick to PvE.

Vanilla. Shortly after launch.

What Windrunner or Darrowmere guilds have you been in?:
None other than my very small "friends-only" Alliance alt-guild. We raided on the Horde side on a different server.

Why did you leave your last guild (if any)?:
Didn't leave, both guilds are inactive now. We raided off and on until Warlords but we lost a lot of members from Cata to WoD.

Unfortunately no.
